D for Diabetes can help with diabetes education to support you with: 

• Managing type 1

• Management of Type 1 Diabetes including bolus calculators and insulin pumps

• Pump starts or pump upgrades/replacement.

• CGM: continue glucose monitoring / flash glucose monitoring (Abbott Libre)

• Carbohydrate counting

• Newly diagnosed Type 1 or Type 2 Diabetes

• Type 2 Diabetes management – lifestyle assessment

• Gestational Diabetes

Fees

A one hour appointment costs $160.

If you have an EPC from your GP then Medicare will reimburse $58.30.

 Insulin Pump Start Packages 

Medtronic 780g with SmartGuard | Tandem t:slim with Control IQ | YpsoPump with CamAPS | Omnipod Dash and Omnipod 5

An insulin pump start package costs $500. This includes the initial 3 hour setup and commencement appointment, a follow up appointment either in person or via telehealth, one month’s worth of email/phone contact as required and all administration costs. The initial appointment is set to 3 hours as you will require education around pump interaction, education around the hybrid closed loop algorithm and possibly setup of the continuous glucose monitor. We’d like to ensure you are competent in your use of the system, and not anxious or uncertain when you leave your appointment.

Referrals 

We accept referrals from endocrinologists and GP’s or self-referred. To get reimbursement from Medicare, you will need an Enhanced Personal Care Plan from your GP (EPC) with diabetes education visits allocated
